Notice to Patients Regarding Non-Covered Services:
We do not fill out disability forms or other paperwork for any organization unless you have been our patient for a minimum of 6 months and have seen you a minimum of 10 times. Even with those limitations, disability is unlikely to be provided and we recommend you see an independent disability psychiatrist for those needs. Any other paperwork filled out will be done at the financial responsibility of the patient, cannot be charged to insurance, and is charged directly to your credit card at the prorated hourly rate.
