Fees

Payment of fees is due at the time of service by cash or credit card. A valid credit card number is required to hold first and one-time appointments. Our providers accept many insurance plans. Please call to see if they are covered under your insurance.

Fee schedule as of March 1, 2018:

Telephone Fees

Telephone sessions are sometimes offered in place of office visits at the discretion of our providers. Fees may also apply to telephone conversations with family members, therapists and other doctors. There is no charge for emergent calls or discussions about routine matters such as appointment scheduling. Matters requiring more than 30 minutes require face to face discussion. Please note that while there are billing codes for these sessions, most insurers do not honor them.

  • 5-15 minutes: $50
  • 20-30 minutes: $100

Administrative Fees

Our providers are solo practitioners and do not employ staff for tasks such as obtaining pre-authorizations and calling in refills. Therefore, they may need to spend a significant amount of time between visits attending to these matters on your behalf.

  • Returned check:  $30.00, bank fees, and original check amount
  • Copying and faxing: $1.00 per page
  • Rates for copying medical records:  $1.00 per page
  • Typed letters/Authorizations: $100 for physician and $75 for therapist and midlevel.
  • Fee level may vary depending on the breadth of the documents. It is reasonable to assess the same fee as telephone fees when the document is time consuming (i.e. 1 hour=$200).

Late Arrivals and Missed Appointments

You will be charged a $100.00 no show fee if you miss or cancel with less than 24 hours (1 business days) notice. You will be called the business day prior to your scheduled appointment to confirm attendance or need to change appointment.  Office staff is not responsible for missed calls and will leave a HIPPA appropriate message when possible. Please remember this is is a courtesy call and does not constitute an excuse for a no show fee exemption.  This means that Monday appointments must be cancelled by the last business day of the week (Thursday).  Approved office holidays that fall on weekdays are not considered business days.

Prescriptions

Office visits are required for all new prescriptions. You are responsible for keeping track of remaining refills and schedule appointments accordingly.
